住所変更/改姓名
◎住所変更をした場合 when change your address
UTASの「学生情報」＞「現住所等変更入力」より連絡先を登録してください。メールアドレスや電話番号が変わった時も随時UTASから情報を更新をしてください。
Please register your contact information in UTAS under 「Student Information」 > 「Change Current Address」. If your email address or phone number changes, please update your information on UTAS.
※注意 Attention※
入学料・授業料免除等申請をしていて、申請後に住所が変わった場合のお手続きについては、以下のサイトの「学費免除FAQ（よくある質問）」をご参照ください。
Please refer to the「Tuition Exemption FAQ (only Japanese)」 on the following website for the procedures when you have applied for the entrance fee or tuition fee exemption or if your address has changed after the application.

◎改姓名した場合 In case of name change
以下の「改姓名届」を、必ず届け出てください。
※在学中旧姓使用を希望する場合も届け出が必要です（旧姓使用をご希望の場合は、改姓名届の空白に「旧姓使用希望」と記入してください）。
届け出の際は改姓が分かる書類（戸籍謄本、書き換えた運転免許証など）の写しを併せてご提出ください。
Please make sure to submit the「Notification of Change in Name」.
※Even if you would like to keep using your maiden name, you need to submit the application. In that case, please write "旧姓使用希望(I want to use my maiden name)" in the blank space of the notification of change of name.
Please submit a photocopy of the document that shows the change of your family name (such as your family register or driver's license) along with your application.
