【１】学位記の氏名の表記を変更したい場合
・「学位記の表記に関する調査票」
を必ず期日までに提出してください。未提出の場合には変更いたしません。
【２】論文公開・許諾書について
論文公開につきましては、柏図書館の管轄です。
