【１】学位記の氏名の表記を変更したい場合
・「学位記の表記に関する調査票」
を必ず期日までに提出してください。未提出の場合には変更いたしません。
【２】論文公開・許諾書について
論文公開につきましては、柏図書館の管轄です。
・「修士論文の公開手続きについて」 を確認の上、以下の問い合わせ先へご相談下さい。

If you would like to change your name on your degree certificate, submit the request form to the Student Affairs Team, GSFS, by the indicated deadline.
※NOTE※
For international students who are not from countries using Kanji characters, the name on the Japanese degree certificate will be written in alphabetical characters.
International students from countries using Chinese characters will continue to have their names written in Kanji characters on the Japanese degree certificates.
Student Affairs Team has only information of the submission schedule.
