【１】学位記の氏名の表記を変更したい場合
・「学位記の表記に関する調査票」
を必ず期日までに提出してください。未提出の場合には変更いたしません。
【２】論文公開・許諾書について
論文公開につきましては、柏図書館の管轄です。

If you would like to change your name on your degree certificate, submit the request form to the Student Affairs Team, GSFS, by the indicated deadline.
※NOTE※
For international students who are not from countries using Kanji characters, the name on the Japanese degree certificate will be written in alphabetical characters.
For international students from countries using Kanji characters, the name on the Japanese degree certificate will be written in Kanji characters.
Student Affairs Team has only information of the submission schedule.
